For this assignment, you’ll be exploring our flagship product—the Advocate Experience.
When a patient comes into Solace, they first have an Intake Session with a Solace physician to explain what’s going on with their health and what their goals are in working with an advocate. Our physicians document their medical concerns in SOAP format and submit an encounter note; we use the contents of this note to match each patient with the right advocate for them.
Once matched with an advocate, the patient schedules their first visit by phone or video. 95% of Solace advocacy is performed virtually.
An advocate’s job is to: Create a Care Plan during the patient’s first visit to meet the needs outlined by the patient and the physician.
Document how they spend their time with each patient. Advocates can expect to spend 5-10 hours per week with each patient, depending on needs.
Create notes and communicate regular updates with each patient to address the patient’s unique needs.
Most importantly: solve patient concerns. Advocates don’t just tell patients how to solve their issues, they actually do the legwork and make solutions happen. They find and call the doctors, they coordinate appointments, they navigate health insurance and endure hold music to get our patients the help they actually need. Our advocates have clinical backgrounds and know the magic words to say—and the right people to say them to—to get things done.
A typical patient story might look something like this: Patient A has been experiencing memory concerns.
Patient is 80+ and currently on Medicare. She considers herself extremely healthy; she has been on a raw food diet for years and has kept a vigilant health diary documenting her daily routines and diet.
For the past year, she’s been misplacing her keys and has even gotten lost while driving, which has led to a growing concern for potential Alzheimer’s.
Patient A lives far away from her adult children, so they’re struggling to help her. She also doesn’t trust her own judgment when it comes to her health.
She reports feeling neglected during her latest visit to her doctor. The doctor spent 5 minutes with her and brushed off her concerns; she hasn’t felt heard.
Patient A learned about Solace through her local senior center and is seeking an advocate who will read her health diary, get a holistic understanding of her symptoms, and help her find a doctor and specialists who can diagnose what’s going on.
